IMO it is more obvious to have reply just doing a personal reply
and have additional functionality in separate entries/buttons.
I would also suggest to sync the custom toolbar stuff and only
show the reply button by default.
I dont have problems using a different key/entry/button for list
replies, as I am aware that I want a list reply rather than a
personal. IMO a button that is named only "reply" but has more
functionality behind, leaves an uncertain feeling.

The reply function shouldnt even be changed by a default-reply-to
option. If reply-to-list can be improved to detect any list (with
a unique header) and overrides reply-to:, the patch can be reverted.
The list detection could be improved by matching a user definable
list of (mailinglist-)headers and their associated to: address.

So my point is: Do exactly what is displayed, avoid hidden magic.
I'd recommend the reply, reply-to-all and reply-to list buttons
for the toolbar, as they represent basically different kinds of
reply (reply-to-sender is only a special case of personal reply).
But syncing the custom toolbar stuff would be the nicest solution.
